Local anesthetics are used for pain control and also to reduce any anxiety during any surgical procedure that is performed. The patient is comfortable and allow the surgeon to perform the procedure with ease. Hair transplantation is a minor surgical procedure. Prior selection and administration of anesthetic agent for surgery is important. The operating procedure is aware of the dose, action, duration and potential toxicity. We use short acting and long acting anesthetic agents, that is lidocaine and bupivacaine that is diluted with normal saline and later adrenaline is added externally to achieve vasoconstriction. Lidocaine is a short acting anesthetic to achieve vasoconstriction. It has a quick onset of action fopr 2 – 4 minutes. The effect lasts for 15 to 30 minutes. The duration of action is longer when combined with adrenaline and can last for about 120 minutes. The recommended total dose of lidocaine is about 300 mg in cases of 2 % anesthesia. The amount can be increased when combined with epinephrine to about 500 mg. Coming to bupivacaine, this a long acting anesthetic agent used commonly in hair transplant. Bupivacaine has a slow onset of action compared to lidocaine and a longer duration of action, about 120 to 240 minutes. Usually for field blocks, 0.25% of bupivacaine is used. For peripheral blocks, we can use a concentration of about 0.5%. Before administering the anesthesia directly in to the scalp, we administer regional blocks, which anesthetizes the nerve that supplies the major portion of the donor and the implanted areas. By giving regional anesthetic blocks, we can reduce the delivery of the anesthesia directly into the tissues. For the donor area, we give a regional block by blocking the lesser and greater occipital nerves and for the frontal hairline and the center, we can block the supraorbital and the supratrochlear nerves. Anesthesia is given using the finest needles in titrated quantities during hair transplantation.

The survival of follicular units upon extraction from the scalp is one of the key variables of successful hair transplantation. If follicular units are transected in the extraction process, there is a greater likelihood that they will not survive the transplant, and the hair transplant will fail. While FUT procedures using strip-harvesting of follicular units typically guarantee a large number of non- transected follicular units, FUE procedures can, and often do, transect grafts, rendering them useless in a transplant.
